I AM AN “OUT-OF-NETWORK PROVIDER” WITH INSURANCE COMPANIES

With your credit card on file (required), I’ll use the private client portal to collect payment on the day of your appointment. If you have health insurance and want to find out if your plan offers “Out-of-Network Benefits” for mental health services, please call them ahead of time to determine this. In some cases, all or part of our sessions may be reimbursed to you. It will also require that I include a psychiatric diagnosis, if you meet criteria (which is the same as in-network coverage).

Some questions to ask your insurance company for Out-of-Network reimbursement:

  • Do I have Out-of-Network benefits for mental health services? If so, what services are reimbursed and at what rate? What is the reimbursement for the intake evaluation (code 90791), the psychotherapy follow-up 53-60min. session (90837) and the psychotherapy follow-up for 45-52min. session (90834)?

  • What is my annual deductible? How much of my deductible have I met so far this year?

  • How and where do I submit a claim?

If you request it ahead of time, I can provide an automatic monthly “Superbill” if you want to seek reimbursement for therapy services. Please note that the Superbill does not guarantee coverage.

Note: I’m not a covered provider for Medicare and they will not allow requests for reimbursement for “opt-out” providers. If you’re on Medicare and want to see me for therapy, let’s discuss how this would work as a private pay arrangement.

I belong to a number of professional memberships and receive referrals from a variety of sources. For example, I am a member of Eastside Psychological Associates (EPA). Please note that EPA is not a group practice, but is a group of independently practicing professionals who share certain expenses, administrative resources and specialty consulation availability. While the members share a name and website, I am completely independent in providing clinical services and I alone am fully responsible for those services. My professional records are separately maintained and no member of EPA can have access to them without your specific, written permission. The billing and payment for my practice is completed by me directly. If you were referred through EPA, thank you for choosing to work with me and please let me know if you have any further questions about my services.

*CANCELLATION POLICY*

I have a 48-hour cancellation policy. For appointments cancelled in less than 48-hours,

the session is billed at the full fee.
